The Kevin Smith incident was with Howard Stern. Stern or his staff called Kevin at some ungodly early hour in LA to get his opinion on some capeshit or other, he's on hold, and Stern makes some remark about Kevin Smith dying to get on the show or begging to be on or something.
That pissed him off and then he migrated to O&A before starting to smoke weed and take pictures of himself crying

I distinctly remember Smith getting pissed off after Opie pulled one of his "Kevin really wants to get in on this" line after the producers had called Kevin. I thought it was why Kevin stopped doing the show.
